We encourage you to fill out your paperwork prior to your appointment—it can help save you time when you check in to our office, spur important conversations between you and your loved ones, and get you thinking about your better hearing goals.
Scheduling Your Appointment
We will see walk-ins if time permits, but it’s always best to call and make an appointment. If your appointment is at least two days out, we’ll mail you the paperwork so you’re stress free on appointment day!
When you call to make an appointment, have these ready:
- Your name
- Address
- Date of birth
- Type of insurance
What You Can Expect During Your First Visit
What to bring:
- Insurance cards
- Driver's license
- A spouse, family member, or friend
Please be early
If you haven’t filled out your paperwork, give yourself 15 minutes to do so before your appointment. Otherwise, we’d prefer you show up 5 minutes before your scheduled appointment time to allow you to get settled in.
Upon arrival, our front office staff will greet you and check you in. We will use the required information to start your chart. We strive to never have our patients wait for a scheduled appointment!
How long is my appointment?
1 to 1.5 hours
What will we cover?
After we greet you and your guest, we’ll tell you a little bit about ourselves, find out important history and information about you, perform a comprehensive hearing evaluation, and recommend treatment options if needed.
